For your convenience, you may print out our patient questionnaire prior to your visit to LaRue & LaRue Pediatrics, PA. Click the New Patient Forms link below, complete the forms and bring them with you to your appointment. Please bring with you any X-rays you may have as well as your insurance cards and Driver's License or picture ID.

Co-payments must be made prior to seeing Dr. LaRue. Payment for appointments/treatment are due when services are rendered. We accept cash, checks and credit cards (Visa, MasterCard, American Express & Discover).  Medicaid is not accepted
